❌ Publix employee Florida Blue plans are not in-network.

Some plans will cover lactation care at 100% and some plans will require cost-sharing. “Cost-sharing” means the portion of the visit fee that you are responsible for paying.

After booking your visit, I will text you with more information about checking your benefits and applying your insurance benefits to your upcoming visit.

❌ I am NOT in-network with Capital Health Plan.

✅ Self-pay fees apply to CHP. 

✅ I encourage CHP members to request Prior Authorization AFTER booking your first visit.  Prior Authorization is a CHP requirement for superbill reimbursement.

✅ I will text you more information about requesting Prior Authorization after a visit is booked.

❌ I do not offer cost sharing or out-of-network billing with CHP.

✅ I offer a sliding scale fee for Medicaid families. All visits are discounted at 50%.

✅ To apply the sliding scale fee, book a self-pay visit and respond “yes” to the question about applying the Medicaid sliding scale fee. Fees are collected at the end of each visit.;

✅ WIC offers lactation support for eligible families.
